Application for Degree - Submit the completed form to the Faculty of Science Student Services Office by September 1, 2014 for the Fall 2014 Convocation, or apply for your degree on Beartracks. 

Degree Assessment Request (BSc General) - Submit the completed form to advisor.science@ualberta.ca to request an assessment of your BSc General Degree. We will return a statement of what courses are remaining in your degree. 

Documentation Request - Submit the completed form to the Faculty of Science Student Services Office if you require a letter or documentation regarding your academic record. 

Nonstandard Course Load - Submit the completed form to the Faculty of Science Student Services Office. You must speak with an advisor for approval. Approval is based on U of A academic standing and GPA. Approval is not automatic. 

Prerequisite Waiver - Fill out the form and obtain signatures from your instructor and the Chair of the Department that is offering the course. Submit the completed form to the Department offering the course. Approval is not automatic. If you need informaiton regarding the instructor teaching the course, contact the Department. 

Statement of Major and Minor (BSc General) - Submit the completed form to the Faculty of Science Student Services Office. Availalbe only for students currently in the BSc General Program. 

Transfer Programs - For students transfering into the Faculty of Science or students transfering to a different program within the Faculty of Science.
